Most employer-based medical insurance plans (such as Anthem and UHC) do not cover routine eye exams. Please check with your plan to see if you have a separate vision insurance plan (such as Blue View Vision, EyeMed, VSP, Davis Vision, etc.) that will be used for your eye exam.
